Equalizer Home Health Services Inc. offers a wide variety of home medical and support care services to meet the unique needs of each individual patient. All of our programs are coordinated by highly trained, experienced and licensed home care nurses whose focus is providing the most appropriate, professional and compassionate care for our patients in a home care setting.
We approach all patients as individuals, carefully assessing all medical conditions and needs before assigning a caregiver. Upon completion, we develop an individualized plan of care and assign a professional that best matches the needs of the client.

Skilled Nursing Care
Equalizer Home Health provides highly skilled, compassionate nurses on a full, part-time or per visit basis, who work closely with the client, their physician, family members and care providers to maintain the client’s health, and prevent further illness and injury. Our nurses conduct patient and environment assessments, and coordinate care with all team members. They provide treatment and educate patients and other caregivers on the disease or injury process, treatment plan, medications, diet and nutrition, foot care, catheter and wound care.
Personal Home Care
Our compassionate, trustworthy and certified Home Health Aids and Personal Care Attendants are available to assist with dressing, eating, grooming, medication reminders and light housekeeping. They are trained in all areas of personal care needs, are CPR certified and have met all requirements for employment, including background checks. They work hand in hand with our nurses and therapists to ensure that each patient’s care plan is carried out according to the wishes of the patient, their family and the patient’s physician. Our personal home care team is here to give you peace of mind, knowing that your loved one is in good hands.
Physical Therapy
Physical therapy assists patients in regaining physical motion and strength. Many therapies can be provided in the home to improve recovery from illness, surgery, or injury. Our licensed physical therapists will evaluate the need for services and create a plan of care that allows clients to develop independence with daily activities. Conditions known to be improved with physical therapy services include, but are not limited to: falls, loss of balance, difficulty walking, joint pain, back pain, hip and knee replacement surgery, and any condition that causes a decline in function.
Occupational Therapy
Home occupational therapy is a rehabilitative service for people with mental, physical, developmental or emotional impairments, that restores or maintains their ability to perform tasks used in daily living, often through developing ways to modify or adapt activities.
After hospitalization or a recent surgery, our Occupational therapists can help you regain your self-care skills right where you live, including fitting for prosthetics and training in the use of mobility assistive devices, fall prevention, customized exercise programs, balance training and motor skills rehabilitation.
Medical Social Worker
Our medical social workers conduct assessments of social and emotional factors related to the patient’s injury or illness, provide counseling based on the assessment, and assist with coordinating community resources. They are an invaluable resource in helping the patient and family cope with the stresses of an injury, illness or major life change, and offer guidance for how to cope with a variety of psychosocial issues, including assistance with practical tasks such as arranging meals and transportation after a hospital stay.
Speech Therapy
Speech therapists work with patients to improve speech, memory and swallowing disorders, and assist staff and family members in communicating effectively with the patient. Equalizer Home Health’s Speech Language Therapists help patients develop their language and speech skills to regain control and increase their communication. Our Speech Therapy program can help with feeding and swallowing difficulties, dysphagia, mild to severe autism, cleft palate, traumatic brain injury, dyslexia, stammering and stuttering, stroke impairments, Alzheimer’s and dementia.
